Patients with resectable stage IIB to IIIB non-small cell lung cancer who received perioperative atezolizumab plus platinum-based chemotherapy achieved a median event-free survival of 62.8 months, compared with 34.9 months for those treated with chemotherapy and placebo — a difference of nearly two and a half years. Beyond the survival data, the atezolizumab regimen produced markedly higher rates of tumor destruction before surgery. Pathological complete response — meaning no viable cancer cells remained in the removed tumor — was achieved in 29.6 percent of patients in the atezolizumab arm versus 8.5 percent with chemotherapy alone. Despite these improvements, the trial did not meet its prespecified threshold for statistical significance on its primary endpoint. The perioperative approach did not compromise patients' ability to undergo surgery. Surgical cancellation rates remained low and similar between the two groups, and no new safety signals emerged. Adverse events were more common during the neoadjuvant phase than the adjuvant phase in both arms, consistent with the combined intensity of chemotherapy and immunotherapy delivered before surgery.news-medical+1
